To support patient care in a remote/hybrid setting, the full-time Florida Licensed Social Worker will assess the psycho/social needs of patients and families, educate them on community resources, and coordinate assistance for various needs such as food, shelter, and healthcare costs. Key responsibilities Assess patients' situations by conducting interviews and gathering medical, psychological, and social information Research and connect patients with community resources, including assistance for pharmaceutical costs Facilitate patient placements to Skilled Nursing Facilities and support care plan coordination with employees Required qualifications Bachelor's degree in Social Work; preference for a Master's in Social Work Proficiency in using Athena Database software, Microsoft Excel, and Word Ability to interpret complex documents and respond to sensitive inquiries Experience in assessing and addressing social determinants of health Strong interpersonal skills for effective communication with patients and community resources
