Supporting a patient panel in the Florida market, the full-time Florida Licensed Registered Nurse will coordinate patient-centered care, manage clinical support, and ensure effective communication and documentation while working remotely. Key responsibilities Coordinate patient-centered care by partnering with providers and care teams to ensure seamless care delivery Collaborate with care team members to develop and communicate care plans, addressing patient needs and goals Manage provider schedules and maintain accurate documentation of patient encounters and clinical interventions Required qualifications Active, unrestricted Registered Nurse (RN) license in the state of Florida Associate's Degree in Nursing (ADN) or higher 2+ years of clinical nursing experience, preferably in primary care or related settings Demonstrated ability to perform clinical triage and manage chronic conditions Proficiency with electronic health records (EHRs) and clinical documentation standards
