To support patient care and navigation, the full-time Florida Licensed Nurse Navigator will triage new patient appointments, assess barriers to care, and educate patients on treatment plans while working remotely. Key responsibilities Triages new patient appointments and facilitates timely scheduling of initial visits and diagnostic tests Educates patients and families about disease processes, treatment options, and wellness strategies Collaborates with a multidisciplinary team to coordinate patient care and improve healthcare policies Required qualifications Bachelor's Degree in Nursing Current Registered Nurse (RN) license in Florida 2-3 years of relevant work experience in a healthcare setting Proficiency in Electronic Medical Record systems and Microsoft Office Experience in patient education and care coordination
