To support Medication Therapy Management (MTM) services, the part-time Clinical Pharmacist will conduct comprehensive medication reviews and patient consultations remotely, requiring a Florida Licensed MTM Pharmacist with strong clinical judgment and documentation skills. Key responsibilities Conduct outbound calls to complete Comprehensive Medication Reviews (CMRs) and provide patient consultations Document patient-specific details, including prescriptions and recommendations, in a third-party web-based application Attend training sessions to enhance role-specific skills and ensure compliance with AHA guidelines Required qualifications Active, valid Pharmacist license in Florida (FL) 4+ years of experience as a clinical MTM pharmacist, with management experience preferred Experience with Medicare Part D Star measures, particularly SUPD, SPC, and CMR Strong knowledge of medical/pharmacy terminology and medication review best practices Tech-savvy with the ability to navigate online tools and applications efficiently
