To support a growing ACO team, a full-time Florida Licensed Clinical Case Manager will provide client-centered services, linking patients and their families to healthcare and community resources while ensuring coordinated access to care, all in a remote work environment. Key responsibilities Conduct assessments, develop care plans, and coordinate services for patients to achieve optimal wellness Advocate for patients and educate them and their families about available healthcare and community resources Document all interactions and services accurately and in a timely manner to ensure continuity of care Required qualifications Active and clear Registered Nurse license in the State of Florida 3+ years of clinical experience, with a preference for 1+ years in case management Strong computer skills, particularly in Excel, and familiarity with EMR systems, preferably Athena Ability to assess and interact with age-specific patient populations Positive, energetic, and well-organized with strong multitasking abilities
