Florida Licensed Case Manager RN

Coordinating patient care from admission to discharge, the full-time Florida Licensed Case Manager RN will monitor care, act as a liaison between patients, families, and healthcare personnel, and develop discharge plans to ensure optimal patient outcomes in a remote setting. Key responsibilities Identifies patient risk factors and evaluates the plan of care through chart reviews and patient interviews Educates patients and families on care progression while facilitating communication between all parties involved Facilitates ongoing insurance authorizations and ensures compliance with regulatory policies Required qualifications Bachelor's Degree in Nursing Florida Registered Nurse (RN) License Minimum of 2 years of relevant experience in case management or healthcare Knowledge of insurance reimbursement policies Experience in developing discharge and transition plans that meet patient needs
