To support patients and families affected by chronic conditions related to aging, the part-time Florida Licensed Care Navigator will provide virtual biopsychosocial support, personalized care planning, and community resource connections while collaborating with a transdisciplinary clinical team. Key responsibilities Act as the primary point of contact for caregivers, families, and patients, enhancing quality of life and safety in the home Conduct biopsychosocial assessments and develop personalized care plans that align with clients' values and needs Coordinate with primary care physicians and healthcare specialists to ensure continuity of care and provide caregiver education Required qualifications Two years of experience in a healthcare or community-based setting preferred One year of experience related to the needs of family caregivers and older adults is ideal Demonstrated understanding of cultural humility and effective communication with diverse populations Familiarity with Electronic Health Records (EHRs) is a plus Proficiency in another language, other than English, is a plus but not required
