Providing essential administrative support, the full-time Care Management Support Coordinator II will perform outreach, answer inbound calls, and schedule services for members while working remotely. Key responsibilities Conduct outreach to members to assist with care plans, resources, and scheduling inquiries Serve as the primary contact for members and providers, addressing inquiries and explaining care plan procedures Document and maintain non-clinical member records in compliance with state and regulatory guidelines Required qualifications High School diploma or GED 1 - 2 years of related experience in care management or customer service Knowledge of health plan activities and community resources Ability to assist with Social Determinants of Health (SDOH) needs Experience with telephonic interactions with members
