We are seeking a Physical Therapist to provide personalized, one-on-one rehabilitative care in patients' homes and communities.
Responsibilities Include
- Practicing independently within an interdisciplinary care team.
- Conducting comprehensive patient assessments to develop tailored treatment plans and monitor progress.
- Delivering rehabilitative therapy adapted to evolving patient needs.
Qualifications
- At least one year of physical therapy experience or successful completion of a probationary period as a Rehab Intern.
- Licensure as a Physical Therapist in the state.
- Valid driver’s license required outside Manhattan.
Additional Details
- Competitive pay range of USD $85,000 to $127,500 annually.
- Per visit compensation varies by region, with higher rates in the Bronx and certain Brooklyn areas.
