You will conduct eligibility visits, perform comprehensive home assessments, and collaborate with a multidisciplinary team to develop plans of care focused on comfort and symptom relief.
The role requires an Ohio NP license, MSN or DNP, DEA, and a valid driver’s license with willingness to travel.
