Everhome is seeking acompassionate and experienced Physical Therapist (PT) to provide in-home therapy services throughout theDownriver area. This role is ideal for clinicians who valueindependence, flexibility, and meaningful one-on-one care.

What You’ll Do
• Complete evaluations, follow-up visits, and discharges in the home setting
• Develop and implement individualized plans of care
• Educate patients and caregivers to promote safety and independence
• Maintain accurate and timely documentation
What You’ll Need
• ActiveMichigan Physical Therapist license
• Minimum1 year of home health experience required
• Strong time management, communication, and documentation skills
• Reliable transportation and ability to travel within the assigned area
