You’ll document visits, monitor safety and well-being, and promptly report changes in condition.
Working in the field, you will coordinate schedules, travel to client homes and community locations, and use electronic systems to record services. You’ll collaborate closely with clinical staff, case managers, and families, and participate in ongoing training.
