💰 Salary: $90,000–$105,000 + $10,000 Sign-On Bonus

🕒 Schedule: Monday–Friday, 8:30 AM–5:00 PM; 50% field, 50% remote

Position Overview

A home care organization is seeking an experienced Registered Nurse Field Supervisor to oversee patient care, conduct in-home supervisory visits, and support field staff across Westchester County.

This position offers a hybrid work arrangement, with one day per week in the office and remote work the remainder of the time, alongside approximately 50–60% field-based visits. The RN will manage a caseload of approximately 25–30 patients and participate in a rotating phone on-call schedule every 4–6 weeks, with additional compensation during on-call weeks.

Key Responsibilities

• Conduct in-home supervisory visits, patient assessments, and follow-up visits in accordance with NYS Department of Health regulations.
• Evaluate home health aides and personal care workers for competency, performance, and adherence to individualized plans of care.
• Identify clinical concerns, safety risks, and gaps in care, communicating findings to the appropriate clinical team members.
• Provide coaching and clinical guidance to field staff during home visits.
• Maintain accurate and timely documentation in the electronic medical record.
• Respond to clinical questions and incidents involving patients, families, and field staff.
• Collaborate with intake, scheduling, and operations teams regarding new cases, transitions, and care plan updates.
• Support quality improvement initiatives, chart audits, and compliance activities.
• Participate in the scheduled phone on-call rotation.

Qualifications

• Active New York State RN license in good standing.
• Associate’s degree in Nursing required; BSN preferred.
• Minimum of 2–3 years of nursing experience, with home care and supervisory experience strongly preferred.
• Current CPR/BLS certification.
• NPI number required.
• Knowledge of NYS home care regulations and supervisory visit requirements.
• Experience with electronic medical records; HHA Exchange and UAS experience preferred.
• Strong clinical assessment, communication, organization, and staff coaching skills.
• Valid NY driver’s license, reliable transportation, and proof of auto insurance.
