Job Summary
The Field Remote Nurse Practitioner/Physician Assistant in the Care Anywhere (CAW) program provides medical and social support to Alignment Health patients in-home, preventing unnecessary hospitalizations and advancing care for seniors and chronically ill patients.
Responsibilities
- Conduct in‑home assessments on Alignment members; review medications and perform health screenings 3.5 days per week.
- Provide patient education and match care to patients’ desired outcomes.
- Identify diagnoses for active medical and care management.
- Partner with the Regional Medical Officer and other team members to develop appropriate care plans.
- Coordinate care with PCPs, specialists, and ancillary providers.
- Lead broader clinical team that may include nurses, health coaches, social workers, and care coordinators.
Job Requirements
- Experience
- Required: None.
- Preferred: One (1) year of prior clinical or home‑care experience; previous EMR experience preferred; experience with older adult (geriatric) patients preferred.
- Education
- Required: Master’s degree from an accredited NP or PA program.
- Specialized Skills
- Effective communication with professionalism and positivity.
- Problem‑solving, organizational and time‑management skills for a fast‑paced environment.
- Licensure
- Required: Active Nurse Practitioner or Physician Assistant license; active RN license and furnishing number (must be provided upon hire).
- NP only: Active Nurse Practitioner Board Certification/Physician Assistant Certification, NPI Number, DEA; Valid BLS.
- Valid Driver’s License and current automobile insurance.
Essential Physical Functions
This job requires regular talking or hearing, standing, walking, sitting, use of hand/finger, handling objects, and reaching. Frequent lifting or moving of up to 20 pounds is required. Adequate vision for close tasks and focus adjustment is needed.
Pay Range
$130,332.00 – $195,498.00, based on market location, education, responsibilities, and experience.
