Field Nurse Practitioners – Multiple Locations

Locations: Oakland CA | Bakersfield CA | Middletown CT | New Haven CT | Willimantic CT | Rockville CT | New London CT | Syracuse NY | Ithaca NY

• Conduct in-home and community-based health assessments
• Address chronic and acute care needs across adult, pediatric, geriatric, and women’s health populations
• Collaborate with patients, specialists, and care teams
• Participate in pop-up clinics and outreach events
• Travel daily within a 60-mile radius (up to 120 miles/day)
• Master’s degree in Family Health (ANCC/AANP accredited)
• Active & unrestricted NP license
• National board certification (ANCC or AANP)
• Valid driver’s license and willingness to travel
• EMR proficiency and strong documentation skills
• Basic Life Support (BLS) certification
• 3–5 years as a Nurse Practitioner
• Home or public health experience
• Experience with immunizations and point-of-care testing
