Field Care Manager Nurse

Humana is seeking a Field Care Manager Nurse to coordinate and deliver high-quality, community-based care for our members. In this role, you will conduct in-home and field assessments, develop and update person-centered care plans, and collaborate with physicians, social workers, and community partners to close care gaps. You will educate members and caregivers on conditions, medications, and preventive care while monitoring progress and adjusting interventions as needed.

Using clinical guidelines and analytics, you’ll advocate for appropriate services, support safe transitions of care, and help improve health outcomes and member satisfaction.
