The role combines in-home assessments with community-based care, maintaining a strong emphasis on safety and quality while enabling independence for members at home.
The position offers flexibility to work from home when not in the field, a schedule of Monday–Friday, 8:00 AM–5:00 PM, and travel up to 75% within the county and surrounding areas.
