Your Health Organization in Charleston, SC is seeking a Registered Nurse Case Manager to support patients across the Charleston area. The nurse will visit patients at homes and facilities, initiate telemedicine visits, and participate in Care Management plans in alignment with laws and policies. Daily travel to communities is required.
The role is full-time with 8-hour shifts Monday–Friday, serving Mt. Pleasant/Downtown Charleston and Summerville/St. George areas.
