Superior Hospice, part of the Home Health Care, Inc. family, is seeking a Hospice Admissions RN to bring peace of mind to families from the first conversation.
You will travel to patient homes and facilities, perform eligibility assessments, and initiate hospice services with the interdisciplinary team. You will lead compassionate discussions about goals of care and Medicare benefits, coordinate seamless transitions, and document accurately in the EMR.
