Field based community health worker

Working Schedule: This position is full time, 40-hour work week, Monday - Friday between the hours of 8:00am - 5:00 pm Central Standard time. Occasionally may work outside these hours due to member's needs. The schedule will be determined by the manager upon hire. This position is a field-based position with a home-based office.

Some of our offerings include: High School Diploma/GED 3+ years of experience working within a community health setting or community outreach experience 2+ years of professional experience working with diverse multi-ethnic and socioeconomic populations 1+ years of field-based experience Intermediate level of computer experience with the ability to navigate a Windows Environment, especially Microsoft Excel Ability to keep all company sensitive documents secure Must live in a location that can receive a United Health Group approved high-speed internet connection or leverage an existing high-speed internet service Required to have a dedicated work area established that is separated from other living areas and provides information privacy Active and unrestricted driver's license and access to reliable transportation enabling you to travel to member homes and communities within a designated area Must reside within a commutable distance to Must be able to plan routes for best time management of schedule and travel time/distance Ability to have flexibility with scheduled hours based on member's availability and/or needs Preferred Qualifications: + years of experience working with vulnerable populations Soft Skills: At United Health Group, our mission is to help people live healthier lives and make the health system work better for everyone.
