Job Definition

Work with the CN Guidance FACT Team to deliver services to those individuals who are diagnosed with severe mental illness and whose needs have not been met by more traditional service delivery approaches.

Educational Requirements

A bachelor's degree or higher in social work, psychology, rehabilitation counseling, or related field preferred.

Experience Required

Two years' experience working with the chronically mentally ill (Masters level) recommended. Three years' experience working with the chronically mentally ill (Bachelor's level) recommended. Competency in substance abuse, family support, or vocational rehabilitation must be demonstrated by at least one year of training and/or experience in integrated mental health and substance abuse assessment and treatment.

Duties And Responsibilities

  • Perform routine team duties and have lead responsibility for integrating dual-recovery treatment, family support or vocational/employment training with the tasks of other team members.
  • Responsible for providing Assessment and Service Planning, Quality Assurance and Improvement Processes, Utilization Review, incident reporting and coordination between FACT and other systems.
  • Engage in assessments, case management, crisis intervention, and discharge planning.
  • Maintain case records and complete documentation on time in compliance with federal, state and agency regulations.
  • Provide direct, Telehealth face‑to‑face and/or collateral services to recipients either in the FACT Office or in the community as needed.
  • Work a 35‑hour work week, including periodic pager coverage arranged in advance with the Team Leader or Assistant Team Leader as needed.
  • Assist recipients in developing appropriate expectations from treatment.
  • Strengthen the recipient’s ability to understand and utilize therapy and find resources after treatment through proper discharge planning and termination processes.
  • Exchange information with other agencies that benefit the recipient, or ensure compliance with regulations and coordination of services.
  • Utilize and understand the value of supervision, which increases practice skills, promotes better services for the recipient and preserves accountability to the agency.
  • Facilitate sharing of data with recipients to support partnered decision‑making and keep clients informed of progress.
  • Complete QI training during onboarding and participate in QI refresher training as needed.
  • Cultivate a culture of inclusion for all employees that respects individual strengths, views and experiences.
  • Adhere to all safety protocols and procedures to maintain a safe working environment for all employees, following established safety guidelines.
  • Demonstrate health and safety awareness through relevant knowledge or experience regarding safety protocols.
  • Other duties as assigned.
