This opportunity combines hands-on patient care with physician and nurse practitioner leadership, supported by structured medical director training and professional development.

• $280,000 $300,000 base salary , plus a bonus opportunity of up to 20% of base

• Partnership opportunity with no buy-in

• Monday Friday schedule; clinic hours 7:30 a.m. 5:00 p.m. , with the last patient scheduled at 4:15 p.m.

• Small patient panel of up to 250 patients

• Care for seniors managing multiple chronic conditions

• Lead and support primary care physicians and nurse practitioners

• Practice in a collaborative environment focused on quality, continuity, and coordinated care

The center offers in-house specialty consultations, including cardiology and podiatry, along with nutrition services, acupuncture, Tai Chi, and yoga. Door-to-door patient transportation through Uber/Lyft helps reduce barriers to care.

• MD or DO with board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine

• At least one year of clinical experience in a geriatric or family practice setting

• Leadership experience preferred

• Strong leadership, communication, and analytical skills

• Proficiency in Microsoft Office

• Ability to travel locally and occasionally regionally

• Spanish language proficiency preferred
