- Location : Remote, but candidates should live near Hendersonville, Nashville, TN.
- Start Date : ASAP
- Full Time No Weekends
- Negotiable Pay/ Weekly Compensation
- Duration/Schedule : 3 months with likely extensions
- Patient Volume : Complete 3+ patient visits daily
Responsibilities
- Maintain quality of care standards
- Assist patients with enrolling in educational programs
- Knowledge of disease diagnosis and prevention
- Assess and evaluate patients' health status
- Implement and follow up on care plans
- Document patient information comprehensively and timely
- Counsel and educate patients and families about health benefits and programs
Requirements
Experience : 2 years of direct patient care relating to polychronic conditions Polychronic conditions, also known as multiple chronic conditions (MCCs), refer to the presence of three or more chronic diseases in a single patient. These conditions can include but are not limited to diabetes, cardiovascular disease, cancer, and more.
