Family Practice/Primary Care Nurse Practitioner

A healthcare organization is seeking an experienced Family Nurse Practitioner to join an employer-sponsored primary care clinic in Hammond, Indiana. This opportunity offers a four-day workweek, low patient volume, and the ability to provide comprehensive primary care in a setting focused on quality patient outcomes rather than productivity metrics.

• Outpatient-only primary care
• Four-day weekday schedule

• Low patient volume averaging 7-12 patients per day
• Care for employees, spouses, and dependents ages 1+

Providers work in a modern outpatient clinic serving employees and their families. Unlike traditional fee-for-service practices, the focus is on preventive care, wellness, chronic disease management, and continuity of care, allowing providers more time with each patient.

• Conduct annual physicals and routine wellness exams
• Diagnose and treat acute and chronic conditions
• Provide primary care services for pediatric and adult patients
• Perform women's health services, including Pap smears and preventive screenings
• Administer immunizations and vaccinations
• Order and review laboratory studies
• Coordinate care with specialists and community providers
• Educate patients on healthy lifestyle choices and disease prevention
• Document encounters utilizing Epic EMR

• Tuesday & Wednesday: 9:00 AM - 7:00 PM
• Thursday & Friday: 6:00 AM - 4:00 PM
• 38 hours per week
• Weekday schedule only
• No weekends
• Start Date: October 2026
• 12-week contract with extension potential

• Average 7-12 patients per day
• RN/MA support available

• Board-Certified Family Nurse Practitioner (FNP)
• Active Indiana Nurse Practitioner License
• Active DEA Registration
• Current BLS Certification
• Minimum 2 years of outpatient primary care experience
• Experience treating patients of all ages
