Family Practice Nurse Practitioner / Physician Assistant
Chowchilla, CA | Full-Time | Permanent | Community Health / FQHC
Join a collaborative primary care team serving a diverse community in Central California. We are seeking an experienced Family Practice NP/FNP or PA to provide comprehensive outpatient care in a team-based setting.
What You’ll Do
- Provide primary and preventive care for adult and family patients
- Evaluate, diagnose, and treat acute and chronic conditions
- Develop treatment plans and coordinate referrals and follow-up
- Perform appropriate office-based procedures
- Maintain accurate clinical documentation
- Collaborate with physicians and other healthcare professionals
- Participate in quality improvement and community health initiatives
What We’re Looking For
- Family Nurse Practitioner (FNP), Nurse Practitioner (NP), or Physician Assistant (PA)
- Graduate of an accredited NP or PA program
- California license or eligibility
- 12+ months of clinical experience preferred
- Primary care or family practice experience preferred
- Strong clinical, communication, and patient-care skills
