Family Physician

Position Overview

One Health Ohio is seeking a Physician to provide direct high‑quality care in a primary care setting within a Federally Qualified Health Center (FQHC). Physicians deliver comprehensive evaluations, diagnosis, and treatment to a diverse population of patients.

Key Responsibilities

• Provide comprehensive medical care, diagnosis, and treatment of acute and chronic illnesses and conditions.
• Educate patients and families regarding diagnoses, treatment plans, preventive care, and safety, ensuring adherence to HIPAA and other regulatory guidelines.
• Collaborate with a team including nurse practitioners, physician assistants and other health‑care providers.
• Collaborate/supervise and mentor mid‑level providers and students as needed.
• Support collaborative, multidisciplinary care with clinicians, behavioral health providers and other team members.
• Provide patient education on health promotion, disease prevention, and management of conditions.
• Provide ongoing follow‑up, continuity care and chronic disease management.
• Maintain accurate and timely documentation in the EHR record system.

Qualifications

• Board certified or board eligible
• Graduate of an accredited school of medicine
• Licensed to practice medicine in Ohio with good standing
• Current DEA certificate
• Completion of an accredited residency program
• Valid Ohio driver’s license
• Current BLS certification

Physical Requirements
