Family Nurse Practitioner

To support a pioneering healthcare team, the full-time Virginia Licensed Family Nurse Practitioner will conduct virtual wellness visits, health assessments, and provide comprehensive care coordination remotely while leveraging modern technology and collaborating with in-home clinical teams. Key responsibilities Conduct virtual adult and pediatric wellness visits, health assessments, and medication reviews Collaborate with in-home team members to evaluate patient needs and arrange necessary care interventions Provide comprehensive care management, including preventive care interventions and referrals to specialists Required qualifications Board Certified as a Family Nurse Practitioner Active Family Nurse Practitioner license in Virginia Experience performing wellness visits and delivering high-quality health services Technologically savvy with proficiency in using digital tools for patient care Ability to work autonomously while maintaining professionalism and accountability
