Position Summary
Terros Health is hiring a full-time Family Nurse Practitioner for a clinic in the Tempe area.
Full-time; 40 hours/week
Schedule: 4-10's or 5-8's

Salary Range:MSN: $115K-$120K+ DOE; DNP: $129K+
Additional Language Differential Pay Available: Spanish,Vietnamese, Korean, Arabic, Farsi, Swahili or ASL:$3/hour ($6240 annually)
Duties include:
- Performs independently or as a part of a treatment team.
- Conducts physicals/check-ups, evaluations, diagnostics, treats illnesses, orders lab tests, general and preventative care, medication prescribing and monitoring, and individual treatment and case consultation.
- Performs the important task of educating patients about preventative care, wellness activities and prescribed treatments.
- Provides leadership and cohesion within the clinic environment, and may provide direct supervision to other professional and/or non- professional medical staff, and students.
- Participates in program evaluation procedures and professional record keeping, makes referrals appropriately for specialty and behavioral health care.
- Provides direct patient services not less than 85% of the work time
- Employs therapeutic interventions appropriate for identified treatment issues
- Seeksappropriate clinical supervision/support, when necessary, to ensure the provision of effective treatment
- Establishes appropriate patient files that conform to agency guidelines, best professional practice and State licensing regulations

Qualifications
- Nurse Practitioner AZ License
- Master’s degree or Doctorate in nursing (Family Nurse Practitioner)
- Possess a valid BLS certification
- Possess a valid DEA license and Board Certification (ANCC)
- Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy
- Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.
- Must pass a Drug Screen, TB Test and Background Check.
- Proficient using computer-based systems including electronic health records (NextGen preferred), Microsoft Outlook, and other Microsoft Office products.
