Family Nurse Practitioner – Full Time

Job Details

• Location: Oak Recovery Health Center, Phoenix, AZ (Near 44th St/Oak)
• Shift: Day Shift, 40 hours per week
• Position Type: Full Time (4‑10
• Required Licenses: Arizona Family Nurse Practitioner license, BLS, DEA, ANCC Board Certification
• Other requirements: valid Arizona driver’s license, Arizona fingerprint clearance (Level1) within 7 working days, drug screen, TB test and background check
• Computer skills: Proficient with electronic health records (NextGen preferred), Microsoft Outlook, and Office products

Compensation

• Salary: MSN $115,000–$120,000 DOE; DNP $129,000+
• Language differential: $3 per hour for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili, or ASL

Responsibilities

• Conduct physicals, evaluations, diagnostics, and treat illnesses
• Order lab tests and prescribe/monitor medications
• Provide general and preventative care with patient education on wellness
• Coordinate with other providers and make specialty care referrals
• Lead and maintain clinic cohesion, including direct supervision of staff and students when necessary
• Participate in program evaluation, professional record‑keeping, and referrals for specialty and behavioral health care
• Spend at least 85% of time providing direct patient services
• Use therapeutic interventions appropriate to identified treatment issues
• Establish and maintain patient files in accordance with agency guidelines, best practice, and state licensing regulations

Qualifications

• Master’s or Doctorate in nursing (Family Nurse Practitioner)
• Valid Arizona FNP license, BLS certification, DEA license, and ANCC Board Certification
• Minimum 4 years of relevant driving experience and valid Arizona driver’s license
• Valid Arizona Fingerprint Clearance card (Level1) within 7 working days of hire
• Pass drug screen, TB test, and background check
• Proficient with computer-based systems (NextGen preferred), Outlook, and Office products
• Experience with NextGen electronic health records is a plus
• 4+ years of Family Nurse Practitioner experience preferred
