Salary Range: $120,000.00

Full-time Family Nurse Practitioner at Community Health Services – 2221 Hayes Avenue in Fremont. Outstanding opportunity for an experienced, compassionate FNP who enjoys caring for a diverse population of patients, including both adults and children.

Applicants must have at least 2 years of experience in a family practice setting.

$120,000 base salary, no RVU's and paid malpractice insurance. As a Federally Qualified Health Center, providers are eligible to apply for student loan repayment through National Health Service Corps (NHSC) – up to $75,000 for full-time employees. Other benefits include paid vacation, sick time, and retirement benefits.

What You’ll Do

• Work directly with patients Monday through Friday – no weekends
• Perform adult and pediatric assessments
• Communicate clearly with providers, patients, and CHS staff
• Follow up on lab and diagnostic testing
• Follow all safety, HIPAA, and regulatory guidelines

What We’re Looking For

• Current license to practice in the State of Ohio as a Family Nurse Practitioner
• Current Basic Life Support certification
• Minimum of two years as a FNP (preferred)
• Knowledge of documenting with EHR
• Strong communication skills and ability to multi-task
• Ability to critically think and problem solve
• Reliable, professional, and compassionate
