amily Nurse Practitioner (FNP) – Full Time

PathCare Health is seeking a full-time Family Nurse Practitioner (FNP) to join our team at our Brooklyn, NY office .

This is a fully in-person position with competitive compensation. We are looking for a reliable, motivated provider who is comfortable working with patients of all ages and managing a steady patient schedule.

Responsibilities:

• Evaluate and treat patients of all ages

• Perform patient assessments and physical examinations

• Diagnose and manage acute and chronic conditions

• Review medications and medical history

• Develop appropriate treatment plans and referrals

• Complete and lock documentation accurately and promptly

• Work closely with our clinical and administrative team

Requirements:

• Active New York State Nurse Practitioner license

• Family Nurse Practitioner (FNP) certification

• Active NPI

• Must be enrolled with New York State Medicaid and eligible to bill Medicaid

• Strong clinical and documentation skills

• Dependable and comfortable working in a fast-paced environment
