Job Title : Family Nurse Practitioner (FNP)
Reports to : Family Medical Doctor / Medical Director

Job Summary
Delivers high-quality primary and preventive care for adults in an integrated community‑mental‑health setting, focusing on chronic‑disease management, wellness promotion, and collaboration with behavioral‑health teams.
Key Responsibilities
- Conduct full health assessments, diagnose & manage acute/chronic conditions.
- Provide medication management, patient & family education, and minor procedures.
- Order/interpret diagnostic testing.
- Collaborate with physicians, therapists, case managers; mentor nursing staff.
- Maintain thorough EHR documentation and participate in quality improvement initiatives.
Qualifications
- MSN or DNP, national NP board certification (AANP/ANCC).
- Active IL NP license; Controlled Substance/DEA & Full Practice Authority preferred.
- ≥12 yrs primary‑care NP experience; behavioral‑health exposure valued.
- Proficiency with EHR (athenaOne) and strong teamwork/communication skills.
Work Environment & Physical Requirements
Ambulatory clinic environment with some standing/walking; must meet standard clinical credentialing and vaccination requirements.
