Practice Info
- Join a group of 15 primary care, 5 pediatric, and 3 OB/GYN providers
- Same Day Surgery Center with visiting orthopedic surgeons
- Site includes infusion center, lab/radiology, Mobile MRI, and CT Scanner
- Visiting specialties include: Cardiology, Nephrology, Oncology, and Pediatric Cardiology
Responsibilities
- Perform history and physical examination
- Admit, evaluate, diagnose, treat and provide consultation to patients of all ages with a wide variety of illness, diseases, injuries, mental health conditions and functional disorders of circulatory, respiratory, endocrine, metabolic, musculoskeletal, hematopoietic, gastroenteric, integumentary, nervous, female reproductive, and genitourinary systems
- Provide pre-operative care of surgical patients
- Provide post-operative medical care of surgical patients
- Address return to work issues

Shift & Schedule
- Monday - Friday 8am-5pm with optional call and half a day of administrative time
Requirements
- M.D. or D.O. degree
- Experience can be demonstrated by providing residency procedure log and a list of completed rotations (for recent residency graduates)
- Experience can be demonstrated by providing a quality profile from hospital(s) where privileges are held, showing clinical activity for the last 12 months (for applicants not directly out of residency)
- Demonstrate active involvement in the clinical aspects of Family Medicine by providing a case log that includes 25 vaginal deliveries, 50 adult medical patients, and 10 inpatient care of children under the age of 18 (excluding normal newborn), as applicable to privileges being requested, in the last 12 months
- Successful completion of a Family Medicine residency program accredited by the ACGME, the American Osteopathic Association (AOA) or the Royal College of Physician and Surgeons of Canada
- Certification in the primary area of practice by the appropriate specialty/subspecialty board of the American Board of Medical Specialties (ABMS), the American Osteopathic Association (AOA), as applicable
- Board Certified/Board Eligible Family Medicine physician
- Applicants not board certified at the time of application but who have completed residency training within the last five years must achieve board certification within five years from the date of completion of their residency training
