Family Medicine Physician
Location
East Jordan, MI (Northern Michigan, near Lake Charlevoix and the Jordan River)
Schedule
Full-time
Clinic Hours
Monday–Friday, 8 AM to 6 PM; Saturday, 9 AM to 1 PM (rotating coverage)
Key Responsibilities
- Provide comprehensive outpatient primary care to patients of all ages.
- Develop individualized care plans, perform physical exams, manage chronic and acute conditions, and coordinate specialty referrals.
- Collaborate with a care team including nurses, behavioral health providers, pharmacists, and care managers.
- Utilize electronic medical records (NextGen).
- Participate in quality improvement, care coordination, and student teaching/mentoring.
- Contribute to the community health mission of our nonprofit, patient-focused center.
Qualifications
- MD or DO with board certification in Family Medicine (or board eligible).
- Licensed or eligible for licensure in the State of Michigan.
- Passion for rural health and improving access to care.
- Experience in community health, FQHCs, or integrated care preferred.
