- Team-based care with RN support for Transition of Care and Chronic Care Management
- Support for addressing identified Social Determinants of Health
- Behavioral health embedded within AdventHealth Primary Care
- Whole-person focus with standardized dementia screenings and coordinated PT
- Age-Friendly Health System certified by the Institute for Healthcare Improvement
- Outpatient only
