Family Medicine Nurse Practitioner/Physician Assistant

Community Care Physicians, a physician-owned multispecialty group in Latham, NY, seeks a Family Medicine Nurse Practitioner/Physician Assistant to provide comprehensive outpatient primary care across the lifespan. In this patient-centered role, you’ll perform exams, manage chronic and acute conditions, order and interpret diagnostics, prescribe medications, and deliver preventive and wellness care. You’ll work closely with collaborating physicians and a robust support team in a culture that values quality improvement, work–life balance, and long-term relationships with patients and colleagues.
