Details and Requirements

• Schedule: Monday – Friday 8a-5p
• Patients per day: 18
• Practice Setting: Federally Qualified Health Center
• Scope: Outpatient Clinic
• State Licensure: Washington state medical license, Basic Life Support (BLS), Board Certified and Drug Enforcement Administration (DEA)
• Credentialing Timeframe: 3 weeks
