As part of its continued investment in data-driven care delivery and risk-based reimbursement programs, the health system is seeking an experienced Epic Healthy Planet Analyst with HCC Risk Adjustment expertise to support strategic population health initiatives.
Position Overview
Our client is seeking an experienced Epic Healthy Planet Analyst with a strong background in HCC Risk Adjustment, value-based care, and population health analytics to support ongoing optimization efforts within its Epic environment. This role will partner closely with Population Health, Clinical Operations, Quality, Revenue Integrity, Risk Adjustment, Care Management, and IT teams to enhance workflows, reporting, and risk capture processes that drive quality outcomes and reimbursement performance.
The ideal candidate will possess deep expertise in Epic Healthy Planet build and configuration, HCC coding workflows, risk stratification methodologies, gap closure programs, and Medicare Advantage risk adjustment initiatives. This individual will play a key role in supporting provider-facing workflows, registries, dashboards, reporting tools, and operational processes designed to improve risk score accuracy and population health outcomes.
Success in this role requires a strong understanding of value-based care models, CMS risk adjustment programs, HCC documentation and coding practices, and the ability to collaborate effectively with both clinical and operational stakeholders across the organization.
Key Responsibilities
- Configure, build, test, and support Epic Healthy Planet functionality aligned with population health and risk adjustment objectives.
- Collaborate with Population Health, Quality, Care Management, Clinical Documentation Improvement (CDI), and Risk Adjustment teams to gather requirements and implement solutions.
- Support HCC Risk Adjustment workflows, including suspect condition identification, gap closure initiatives, provider outreach, and documentation improvement efforts.
- Design, configure, and optimize registries, workqueues, dashboards, and reporting tools within Healthy Planet.
- Analyze risk adjustment processes and recommend workflow improvements to increase HCC capture accuracy and quality measure performance.
- Participate in system enhancements, upgrades, optimization projects, and regulatory initiatives related to value-based care programs.
- Develop and execute testing plans, validation scripts, and implementation activities for Healthy Planet enhancements.
- Investigate and resolve application issues while performing root cause analysis and recommending corrective actions.
- Support end-user adoption through workflow documentation, training materials, and knowledge transfer sessions.
- Collaborate across Epic applications, including Healthy Planet, Ambulatory, Cogito, Reporting Workbench, SlicerDicer, and Care Management modules.
- Maintain detailed documentation of application configurations, workflows, build decisions, and support activities.
- Ensure adherence to Epic best practices, organizational governance standards, and change management processes.
Required Qualifications
- Epic Healthy Planet Certification required.
- Minimum of 6+ years of Epic Analyst experience supporting healthcare organizations.
- 3+ years of hands-on experience supporting HCC Risk Adjustment, Medicare Advantage, or value-based care initiatives.
- Experience configuring and supporting Epic Healthy Planet registries, dashboards, reports, and workflows.
- Strong understanding of CMS-HCC methodologies, RAF scoring, quality measures, and risk adjustment programs.
- Experience supporting population health operations, care management, and quality improvement initiatives.
- Proven ability to work closely with clinical, operational, and executive stakeholders.
- Experience with system configuration, testing, troubleshooting, and production support.
- Strong analytical, problem-solving, and critical-thinking skills.
- Excellent communication, documentation, and stakeholder engagement abilities.
- Ability to independently manage priorities and deliver results in a fast-paced environment.
Preferred Qualifications
- Additional Epic certifications such as Ambulatory, Care Management, Cogito, or Population Health-related certifications.
- Experience supporting Medicare Advantage, ACO, MSSP, CIN, or other value-based care programs.
- Experience with provider documentation improvement and clinical quality initiatives.
- Previous Epic implementation, optimization, or expansion project experience.
- Experience supporting large, complex, multi-facility health systems.
- Familiarity with risk adjustment analytics platforms and population health reporting tools.
- Consulting experience supporting healthcare organizations through population health transformations.
- Experience participating in enterprise-wide value-based care or digital health initiatives.
