- Evaluate, diagnose, and manage patients with diabetes and other metabolic disorders
- Perform comprehensive patient assessments and follow-up care
- Order and interpret laboratory and diagnostic studies
- Develop individualized treatment plans
- Prescribe and manage medications, including insulin therapies
- Educate patients on disease management and lifestyle modifications
- Assist with insulin pump and continuous glucose monitoring (CGM) therapies
- Monitor patient progress and adjust treatment plans as needed
- Maintain accurate and timely clinical documentation
- Collaborate with physicians and clinical staff to provide high-quality patient care
- Graduate of an accredited Physician Assistant program
- Active New York Physician Assistant license
- NCCPA certification
- Minimum 3 years of relevant clinical experience
- Experience managing patients with diabetes and chronic conditions
- Strong communication and patient education skills
- EMR proficiency
- Previous specialty practice experience
- Insulin pump management experience
- Continuous Glucose Monitoring (CGM) experience
- Strong interest in chronic disease management and patient outcomes
