Facility Information:

• Expected to serve over 8,000 patients annually
• 24 hour physician coverage daily
• 12 exam rooms
• Meditech P-doc EMR & M-Modal Dictation
• State of the art imaging, including CT, Ultrasound, and Radiology
• Patients needing to be admitted to an inpatient bed will be transported to the main campus with seamless care handoff to our excellent intensivists, hospitalists, and specialists.

Blake is a 383-bed, Level II tertiary hospital offering a wide range of services including the area's only Trauma Center, 24-hour emergency care, open-heart surgery, cancer care, joint replacement and inpatient rehabilitation. Blake has been named a Top 100 Hospital 9 times by Solucient.
