Qualifications

• Must have active Indiana Medical License and DEA
• Experience: 5 Years
• Board Certified: Emergency Medicine (prefer ATLS)
  • No Family Medicine Board Certifications
  • Internal Medicine Board Certification with ATLS and experience will be considered
• ACLS/BLS
