Job Description

Location: 50 Hospital Drive, Hendersonville, NC Suite 5A Shift: Mon-Fri 8am-5pm Assesses patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and review of medical records and clinical data. Develops and implements patient care plans in collaboration with the interdisciplinary team, observing outcomes in accordance with nursing standards. Demonstrates competency in therapeutic and diagnostic interventions, including IVs, wound care, medication administration, catheterization, and specimen collection. Advocates for and protects the health, safety, and rights of each patient. Provides patient and family education based on assessment of learning readiness, needs, and ability, and evaluate the effectiveness of teaching. Encourages patient engagement in healthcare through motivational interviewing, self-management protocols, and teach-back methodology. Coordinates and implements group visits for identified high-risk patient populations. Aids in the care transition of high-risk patients, including medication reconciliation and coordination of services.
