Supporting the Enhanced Care Management (ECM) program, the remote ECM Care Coordination Specialist will assist members in navigating healthcare and community-based services, coordinating appointments, and maintaining communication among members, Care Managers, and providers. Key Responsibilities Conduct outreach to assigned ECM members and assist with engagement in the program Coordinate medical, behavioral health, and community-based appointments for members Follow up with members regarding their care needs, appointments, and resources Required Qualifications Experience in care coordination, case management, or related fields Familiarity with community resources and social service programs Ability to manage multiple assignments and follow-up responsibilities Experience documenting client or member contacts and service coordination activities Bilingual or multilingual communication skills based on the needs of the populations served
