Description

• Conduct evaluations of patients in both inpatient and outpatient settings, creating tailored treatment plans
• Implement physical therapy interventions centered on mobility, strength, balance, and functional abilities
• Collaborate effectively with nurses, physicians, case managers, and allied health professionals to enhance patient outcomes
• Provide education to patients and their families regarding therapeutic goals, discharge protocols, and at-home exercises
• Meticulously document evaluations, treatment plans, and patient progress in compliance with facility standards and regulations
• Participate actively in care team meetings and interdisciplinary discussions to ensure aligned care for patients

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized credentials (e.g., neurological, geriatric) are appreciated but not required

Skills

• Previous clinical experience is highly desirable
