In this role, you’ll collaborate with physicians, case managers, and interdisciplinary teams to assess readiness for discharge, complete clinical documentation, reconcile medications, and educate patients and families on post-hospital care. You’ll use EHR systems, community resources, and evidence-based practices to reduce readmissions and improve outcomes.
