We are committed to providing compassionate, high-quality care while creating a supportive environment for our staff.We are seeking an experienced Discharge Planning Coordinator (LPN/CSW) to oversee the discharge process for our Sub-Acute Unit and help residents transition safely and successfully to the next level of care.Responsibilities of a Discharge Planning Coordinator (LPN or CSW):Coordinate and oversee resident discharge planning for the Sub-Acute UnitParticipate in utilization review and insurance/Medicare reviewsIssue Medicare cut letters as neededCollaborate with interdisciplinary teams on treatment and discharge plansProvide social service support to residents and familiesCoordinate community resources and post-discharge servicesMaintain accurate records and documentationQualifications:Active NJ Practical Nurse (LPN) or Social Work licenseMinimum 4 years of social work or clinical nursing experience, including admissions/discharge planningLong-term care or rehab experience preferredWhat We Offer:Flexible scheduling opportunitiesOutstanding county benefits packageSupportive team environment with growth opportunitiesRoosevelt Care Center at Old Bridge is proud to be an Equal Opportunity Employer.
