Discharge Planning Coordinator (LPN/CSW)

We are seeking an experienced Discharge Planning Coordinator (LPN/CSW) to oversee the discharge process for our Sub-Acute Unit and help residents transition safely and successfully to the next level of care.
Responsibilities of a Discharge Planning Coordinator (LPN or CSW):
- Coordinate and oversee resident discharge planning for the Sub-Acute Unit
- Participate in utilization review and insurance/Medicare reviews
- Issue Medicare cut letters as needed
- Collaborate with interdisciplinary teams on treatment and discharge plans
- Provide social service support to residents and families
- Coordinate community resources and post-discharge services
- Maintain accurate records and documentation
Qualifications:
- Active NJ Practical Nurse (LPN) or Social Work license
- Minimum 4 years of social work or clinical nursing experience, including admissions/discharge planning
- Long-term care or rehab experience preferred
