Job Overview

The Discharge Planner provides services to patients, families, and others in all hospital areas specific to assignment as applicable, and in accordance with hospital customer service expectations. Professional services to be provided to meet the discharge planning needs of the patient may include, but are not limited to, case consultation, patient education, resource management, and assistance with discharge planning.

Responsibilities

  • Acts as an integral part of the interdisciplinary team, providing community outreach, networking, and liaison with community programs as needed.
  • Consistently applies infection control policies and practices.
  • Meets population‑specific competencies.
  • Attends department‑specific education/training, in‑services, and staff meetings.
  • Contributes to department‑specific performance improvement projects.
  • Provides timely and appropriate discharge planning assessments and interventions for patients.
  • Develops and implements appropriate discharge planning plans of care.
  • Advocates for patient rights.
  • Knowledgeable regarding community resources and mobilizes them on patient’s behalf.
  • Maintains appropriate documentation and statistical records.

Qualifications

  • Bachelor’s degree from an accredited school of Social Work or related field (preferred); RN or LVN (preferred).
  • Two years of acute or related medical care experience.
