Lutheran Social Services (LSS) seeks a Disability Services Consultant to support individuals with disabilities in accessing resources, housing, behavioral health, and community-based services. In this mission-driven, client-centered role, you will complete assessments, develop individualized service plans, and provide case management, advocacy, and education to promote independence and inclusion. You’ll collaborate with interdisciplinary teams and community partners, connect clients to benefits and supports, and maintain accurate documentation while living out LSS’s values of dignity, compassion, and social justice.
