Director of Quality Assurance

The Quality Assurance (QA) Director is responsible for the Agency’s overall quality assurance including the Agency’s Incident Investigation process. The QA Administrator serves as a member of the senior management team and works under the general direction of the Executive Director.

ESSENTIAL FUNCTIONS:

Review all incident reports/investigations prior to the Special Incident Review Committee (SIRC) meetings, anticipate and resolve any issues, questions prior to the meeting. Actively participate as a member of the SIRC. Provide follow-up information when requested by the committee and/or Executive Director.

Maintain internal system for monitoring recommendations and corrective actions for serious reportable incidents, allegations of abuse, reportable and minors. Manage data and provide reports to the Executive Director as requested.

Ensures proper classification of incidents and timely notification to oversight entities.

Provide in-service training on 624 regulations and incident reporting as needed/requested by senior management.

Trains agency management to become agency investigators

Monitor trend analysis quarterly and annually, as well as monitor and report trends evident that may develop throughout the year.

Serve as liaison with DDSO, CQC and MHLS by responding in a timely manner to requests for information. Provide monthly status to DDSO of all open cases. Assure notification of closure of cases to CQC and MHLS. Provide clarification/follow-up to investigator regarding any reopened cases.

Under special circumstances, as determined by the Executive Director, investigate possible abuse or serious reportable incidents. Complete comprehensive report for SIRC review.

Maintain current body of knowledgeof current regulations. Provide assistance/training on applicable regulations, when requested. Review any drafts of new regulations and develop comments/response to the Executive Director. Attend relevant hearings/seminars/workshops

Responsible for implementation of the Quality Assurance Plan.

Special Incident Review Committee (SIRC); Chair of the Agency Minor/Reportable Committee); Quality of Care Committee (QCC); Chair of the Agency Quality Assurance/Quality Improvement Committee (QA/QI). Chair of the Agency Speakers Bureau Committee Participate in site surveys and internal monitoring as it relates to QA/QI. Serve on the Agency Safety Committee in conjunction with the Human Resources Administrator.

Attend at all Board of Directors meetings. Report to Board on activities of the QCC, QA/QI, committees.

Other duties as assigned by the Executive Director.

MINIMUM QUALIFICATIONS:

Bachelor’s Degree in Human Services or related field; working knowledge of state and federal regulations as they relate to MR/DD population; At least 5 years experience working with MR/DD population, with at least 2 years in an administrative/management role. A New York State Driver’s License is required.
